To get the Elf Girl's heart you have to use the +5 cutpurse daggar on the 11th level of the Bothwar dungeon.

I find it much easier just to keep using Ice Bolt over and over, though. Organ removing is such a pain, and it wears down weapons fast. Unless there is a necromancer in the area, I wouldn't bother with the dismemberment.
